Read the first couple comments...
It was believed to be that the gun was loaded for hunting season and never shot, then stored in a closet for a few months. when it was taken to the range the first load was forgot about and then we reloaded it. it was double charged.

What idiots. I would agree and say it was probably double loaded. Carelessness with guns sooner or later ends badly. I have a notch on my ramrod to show a loaded level.
