The most common problem, normally, is the thumb screw on the back of the 'pull to cock', or the secondary safety, is not screwed to the rear as far as possible.So the next check is to see if the plunger is dropping the full length. The plunger should actually rest on the nipple, but you really can not see this because of the hood on the face of the plunger.

(does not change) have it screwed all the way out
