I received one. I'm a marketing guy (not at RMEF...), so for those not in that profession, let me tell you that the survey is a customer profile initiative. Customers who would voluntarily fill that survey out are pure marketer's gold! That's not to say it's bad. If you like getting targeted advertisements then it's all good.
If I have to see an advertisement, I'd rather see one I'm interested in than one for tampons. But that's just me.

For my part, I'll keep the address stickers and notepad and junk the rest.
