I have a CVA Cascade rifle in .22-.250.  I have an Axeon 4-16x50mm, Dog Soldier Scope, 30mm tube.  Which 30mm Talley Scope mounts should I go with?  Low, low extended, medium, medium extended, high or high extended?

To be safe and not have to return, use the high extended. They are only 1/10 of an inch higher than meds.
The extended will allow you to move the scope forward or rearward a little more or less.
The only way to know which set is perfect is to have them both present.

I put one on my tikka and I think medium heigh rings, worked perfect for 50mm scope
