looks like a s&w 686 in 357 the bullets look to be hrdy 125 jhp the same i shoot you got to be careful with the 357 one of a few guns you can doubble charge and have that happen thats why when i load for mine i always work one bullet at a time or that can happen

I don't think they went off (ignited) just got tore up in the explosion.

I'm careful and I've still forgot to move the funnel and tried to double charge a case, in my case I've been lucky enough that it always happened when loading a slow powder in a rifle case that couldn't hold a double charge so I've always caught it.
Here's a pic of a friends ruger 45LC after firing a double charged cartridge.

yep it is a S&W M629 44 mag i thought it was a 686 they looks twin and it says 44 on the barrle didn't look close enough. 357 mag 44 mag 2 of the cases that it can be done to. i have to watch because i shoot 357 mags and my 454 casull both can be doubble charged if one is not paying attenstion i measure one load seat one bullet then go on when loading for those 2 cals

Here's the text that came in the email with the pics:
WONDER WHAT THAT LOAD WAS!
A guy came into our department the other day to ask a favor. He had a S&W 629 that he wanted to dispose of after a mishap at the range. He said there was a loud bang when he tested his new load and the gun smacked him in the forehead, leaving a nice gash. When the tweety birds cleared, this is what he saw.....
