Strange malfunction

I was shooting my 9mm Beretta Nano yesterday when I experienced a fail to feed. I pulled the mag and racked the slide back slowly and to my surprise the fresh round was stuck behind the spent casing which was still in the chamber . . . the wrong way!  Only a eighth of an inch of the case was exposed. So I put in an empty mag (to keep the slide open) and gently tapped the case with a short cleaning rod and it easily came out, much to my relief. 

In my nearly 40 years of shooting pistols I can't recall a semi auto ever doing this. Anybody else seen this type of thing happen?
