liability release form

Does anyone have a copy of a good release form for land owners?  I saw one on this site somewhere but could not relocate it to print it out.  I am hunting mostly "Hunt by writen permission only and would love to have something to take to the land owners to help them feel better about hunting.

Kind of generic but I think this would work.

If you are hunting on written permission only land the landowner should have cards to fill out from the wdfw to give to you
