I don't believe it is required but, I'd need to double check. it must be microchipped or tatoo'd.

Depending upon what breed you have will depend upon what eye testing you might do.

For labs, all breeders should be getting a CERF test done which is visual and PRA which is a DNA test. All breeds can be CERF'd and should be done every year if breeding is in your plan. CERF costs roughly $50 or less. PRA test is about $150.

Got to the OFA website and it has lengthy discussion and information on the subject. Basically, you go to a vet who does OFA films, get it done, then, fill out the paperwork, write another check to OFA with your application and send it off for review by the OFA board. They'll then score your dogs' hips.
"Far better it is to dare mighty things, to win glorious triumphs, even though checked by failure, than to take rank with those poor spirits who neither enjoy much nor suffer much, because they live in the grey twilight that knows not victory nor defeat."
Theodore Roosevelt 1899
